The form can be scanned and sent to the email address indicated at the bottom of the form. You do not have to send the original, but you must sign before scanning.
If payingwith the travel account, the card number (requested in the claim form) can be obtained from the department / unit's finance executive officer or from the Amex Corporate Card customer centre (22 96 08 00 or email@example.com.)

If the trip has been paid for by other credit cards, for example VISA, Master Card or Diners Club, the insurance conditions set by the card in question apply, and you must contact your own card company.
In connection with cancellation due to illness, all insurance companies usually require a medical certificate to be attached.
